Precautions for taking erlotinib: hyperphosphatemia, stomatitis, nail peeling, and eye toxicity
During the treatment with adatinib, hyperphosphatemia, stomatitis, nail dissection, and ocular toxicity are four adverse reactions that require focused monitoring and management. Proactive prevention and timely treatment help ensure the continuity of treatment, as explained below.
1、 Hyperphosphatemia
Almost all medication users will experience elevated blood phosphorus levels, and regular monitoring of blood phosphorus levels is necessary from the beginning of treatment.
Adjust subsequent doses or suspend medication based on blood phosphorus levels to avoid the risk of ectopic calcification caused by long-term high phosphorus.
Moderately limit high phosphorus foods (such as dairy products and animal organs) in diet, but do not use phosphorus binders on your own and follow medical advice.
2、 Oral inflammation
It can manifest as redness and swelling of the oral mucosa, painful ulcers, or difficulty eating.
It is recommended to use a soft bristled toothbrush and non-alcoholic mouthwash to maintain oral hygiene.
Avoid spicy, overheated, or hard foods; If the pain is obvious, recommended oral care solution or local pain relief measures by a doctor can be used.
3、 Nail peeling and paronychia
Nails or toenails may detach from the nail bed, discolor, or experience redness, swelling, and pain around them.
It is recommended to keep hands and feet dry and avoid trimming nails too short or damaging the skin around the nails.
Wear loose and comfortable shoes and socks to reduce pressure on toenails; Seek medical attention promptly when there are signs of redness, swelling, or infection.
4、 Ocular toxicity
May cause central serous retinopathy or retinal pigment epithelium detachment, manifested as blurred vision, dark spots, or visual distortion.
Regular ophthalmic examinations (including vision, slit lamp, OCT, etc.) are required before and during treatment.
If any new or worsening visual symptoms occur, medication should be stopped immediately and eye care should be sought as soon as possible to avoid irreversible visual impairment.
