The use of adatinib requires strict screening of the appropriate population, and some patients may face higher risks due to their underlying conditions. Patients with severe liver and kidney dysfunction and active eye lesions require careful evaluation, as explained below.

1、 Severe liver dysfunction patients

Patients with moderate to severe liver function impairment (Child Pugh B/C grade) should avoid using erlotinib as impaired drug metabolism may increase the risk of systemic toxicity.

Before medication, comprehensive liver function indicators (including bilirubin, transaminase, etc.) should be tested to determine the liver function grading.

Mild liver dysfunction patients can take medication under close monitoring, but the dosage may need to be adjusted individually.

2、 Severe renal insufficiency patients

Patients with end-stage renal disease or requiring dialysis treatment are not recommended to use erlotinib due to a lack of sufficient safety data support.

Before treatment, the estimated glomerular filtration rate should be evaluated. If it is less than 30mL/min/1.73m ², the pros and cons should be weighed or alternative options should be chosen.

If renal function continues to deteriorate during medication, treatment should be promptly interrupted and the cause investigated.

3、 Active eye disease patients

Individuals with a history of central serous retinopathy or retinal pigment epithelial detachment have a significantly increased risk of medication use.

If there is uncontrolled increase in intraocular pressure, macular edema, or optic neuropathy, ophthalmic evaluation should be conducted first before deciding whether to use it.

During the treatment period, if there is a new onset of blurred vision, dark spots, or visual distortion, the medication should be stopped immediately and an ophthalmic examination should be sought.
