Indications and medication precautions of Nebivolol for Macleods Pharmaceuticals in India
Nebivolol is a highly selective beta 1 receptor blocker with vasodilatory effects, mainly used for the treatment of hypertension and chronic heart failure. Attention should be paid to heart rate monitoring, liver and kidney function adjustment, and medication contraindications for special populations.
1. Indications
(1) Hypertension
Suitable for the treatment of adult primary hypertension, it works by reducing peripheral vascular resistance and mildly reducing cardiac output.
(2) Chronic heart failure
Used as an adjuvant therapy for stable chronic heart failure (NYHAII-III grade), it needs to be used in combination with other heart failure drugs.
2. Medication precautions
(1) Dose adjustment
The initial dose for hypertension is 5mg once daily, with a maximum dose of 40mg; for heart failure, the starting dose should be 1.25mg, doubling every 2 weeks to the target dose of 10mg.
(2) Cardiovascular monitoring
During the initial stage of treatment, heart rate/blood pressure should be monitored. If the heart rate is below 50 beats per minute or if atrioventricular block occurs, medication should be reduced or discontinued.
(3) Liver dysfunction
Child Pugh B/C patients are prohibited from using; Mild liver dysfunction patients should start with 2.5mg and titrate cautiously.
(4) Renal insufficiency
When eGFR<30mL/min, the maximum dose is 10mg/day, and it is contraindicated for hemodialysis patients.
(5) Special populations
Pregnancy may cause fetal bradycardia, and breastfeeding may be suspended; Elderly patients should start from the lowest dose.
