Attention should be paid to infection risk monitoring, specific population contraindications, and drug interaction management when using Abrotinib for the treatment of atopic dermatitis. The precautions include infection control, cardiovascular assessment, and medication for special populations. Drug interactions mainly involve CYP2C19 enzyme metabolism drugs and immunosuppressants.

1. Medication precautions

(1) Infection risk: It may increase the risk of opportunistic infection such as herpes zoster. Before use, it is necessary to screen for hepatitis B virus and tuberculosis infection, and drug administration is suspended during active infection.

(2) Cardiovascular monitoring: may cause elevated blood lipids. Blood lipid levels should be tested at baseline and 4 weeks after medication, and lipid-lowering therapy should be initiated if necessary.

(3) Abnormal liver function: Treatment should be interrupted when ALT/AST>3 times the normal value, and dosage should be adjusted for patients with moderate to severe liver injury.

(4) Thrombosis prevention: Long term bedridden or postoperative patients need to assess the risk of venous thrombosis and take preventive measures if necessary.

2. Drug interactions

(1) Strong CYP2C19 inhibitors, such as fluconazole, may increase the blood concentration of abcitinib, and the dose should be reduced to 50mg once daily.

(2) Immunosuppressants: Combination use with JAK inhibitors or biologics may increase immunosuppressive effects, and combination use is prohibited.

(3) CYP2C19 inducers: If rifampicin may reduce drug efficacy, alternative treatment options should be considered.

(4) Live vaccines: Avoid receiving live vaccines during treatment, and inactivated vaccines should be administered at intervals between medication.
