Adagrasib usually requires several courses of treatment
The treatment schedule of Adagrasib needs to be personalized based on the treatment response and tolerance of KRAS G12C mutant non-small cell lung cancer patients. It is usually based on the principle of continuous medication until disease progression or intolerable toxicity occurs, with no fixed treatment period limit.
1. Basic medication plan
(1) Standard dose: Take 600mg orally daily (2 tablets 300mg), once every 12 hours (total daily dose 1200mg), and swallow the entire tablet.
(2) Duration: Clinical studies have shown a median treatment duration of 8.2 months, and the actual course length depends on the tumor response.
Principle of dose adjustment
(1) Reduction strategy: When grade 3 or above adverse reactions occur, the dosage can be gradually reduced to 800mg or 600mg per day, and each reduction should be evaluated for at least one week.
(2) Suspension criteria: Discontinue administration when serious side effects such as QTc interval>500ms and liver toxicity (ALT/AST>5-fold ULN) occur.
3 efficacy evaluation nodes
(1) First assessment: CT/MRI examination will be conducted 6-8 weeks after medication to confirm tumor response, followed by follow-up every 2-3 months.
(2) Continuous benefit: Patients who achieve partial remission or disease stability should maintain treatment until imaging confirms progression or clinical deterioration.
4. Management of special groups
(1) Patients with liver damage: Child Pugh B grade should be reduced to 600mg/day, while C grade should not be used.
(2) Elderly patients: For those aged 75 years or older, it is recommended to strengthen electrocardiogram and liver function monitoring without the need for routine dose adjustments.
