Eltrombopag, as a thrombopoietin receptor agonist, is suitable for the treatment of chronic liver disease-related thrombocytopenia and chronic immune thrombocytopenia (ITP) in both adult and pediatric patients. The dosage should be adjusted according to specific indications and platelet counts should be strictly monitored.

Chronic liver disease associated thrombocytopenia

(1) Preoperative application: For adult patients with chronic liver disease who plan to undergo invasive procedures, treatment should be initiated 10-13 days before surgery, and a daily dose of 40mg or 60mg should be selected based on baseline platelet count, with continuous medication for 5 days.

(2) Applicable standards: Mainly applicable to Child Pugh A/B patients, severe liver dysfunction (Child Pugh C) requires careful evaluation of efficacy and thrombus risk.

Chronic immune thrombocytopenia (ITP)

(1) Adult therapy: Used for persistent ITP patients with poor response to first-line treatments such as cortisol and immunoglobulin, with an initial dose of 20mg/day, adjusted based on platelet response.

(2) Children's treatment: For children over 6 years old, 20mg tablets per day are used, and for children aged 1-6 years old, 10mg powder is used per day. Both require maintaining platelet count ≥ 50 × 10 ⁹/L to reduce the risk of bleeding.

3 Treatment restrictions for special populations

(1) Pregnancy: Lack of safety data, potential fetal risks need to be weighed.

(2) Breastfeeding period: Animal experiments have shown that drugs can be secreted into breast milk. It is recommended to suspend breastfeeding during the treatment period and for 28 days after discontinuing the medication.

(3) Liver and kidney dysfunction: Moderate to severe liver dysfunction requires dose adjustment, and data on patients with severe renal dysfunction is limited.
