The correct use of eltrombopag requires strict adherence to indications and individualized dosage requirements. Key points include co administration with food, fixed time administration, and dynamic adjustment of dosage based on platelet count. At the same time, attention should be paid to missed doses and contraindications for special populations.

1. Routine administration method

(1) Medication time: Take orally at a fixed time every day, and it is recommended to take it with meals to reduce gastrointestinal reactions. A high-fat diet does not affect the efficacy of the medication.

(2) Dosage selection: Adults and children over 6 years old should use tablets, while children aged 1-6 years old should use powders. Both should be taken in whole tablets/granules and should not be chewed.

2 Dosage Adjustment Standards

(1) Initial dose: 20mg/day for adults and children over 6 years old with chronic ITP, 10mg/day for children aged 1-6 years old, and 40mg or 60mg for patients with chronic liver disease based on platelet count before surgery.

(2) Titration principle: Evaluate platelet count every 2 weeks. When the platelet count is below 50 × 10 ⁹/L, increase the dose; when it is above 200 × 10 ⁹/L, decrease the dose. The maximum daily dose should not exceed 40mg (adults) or 20mg (children).

3 Omission and Overtreatment

(1) Remedial measures for missed doses: Take the medication immediately upon discovery, but do not double the dosage. Continue to administer the medication according to the original plan the next day.

(2) Overdose response: There is no specific antidote yet, and immediate medical attention is needed to monitor platelets and receive symptomatic supportive treatment.

4 Special precautions

(1) Monitoring requirements: Platelets should be tested weekly during the initial stage of treatment, monthly after stabilization, and continue to be monitored for 4 weeks after discontinuation of medication.

(2) Contraindications: Prohibited during pregnancy, feeding should be suspended during lactation, and caution should be used with severe liver and kidney dysfunction.
