Eltrombopag is an oral thrombopoietin receptor agonist primarily used to treat chronic liver disease-related thrombocytopenia and chronic immune thrombocytopenia (ITP). It stimulates the proliferation and differentiation of bone marrow megakaryocytes to increase platelet count, making it suitable for both adult and pediatric patients.

Chronic liver disease associated thrombocytopenia

(1) Preoperative application: For adult patients with chronic liver disease who plan to undergo surgery, medication should be started 10-13 days before surgery, and the dose should be adjusted according to platelet count (40-60mg/day) for 5 consecutive days.

(2) Liver function grading: applicable to Child Pugh A/B patients, and those with severe liver function impairment should be carefully evaluated for the risk of thrombosis.

Chronic immune thrombocytopenia (ITP)

(1) Adult treatment: As a second-line medication used for patients with poor response to cortisol/immunoglobulin, the initial dose is 20mg/day, and platelet count needs to be monitored regularly.

(2) Children's treatment: The initial dose for children over 6 years old is 20mg/day, and for children aged 1-6 years old, 10mg/day of powder is used. Both require strict monitoring of platelet levels.

3 Medication precautions

(1) Thrombosis risk: Special attention should be paid to patients with a history of thrombosis or genetic thrombophilia. If symptoms such as swelling/chest pain occur, immediate medical attention should be sought.

(2) Special populations: contraindicated during pregnancy, medication should be discontinued for more than 28 days during lactation, and individualized evaluation is required for medication in elderly patients.
