Iprenone (Inspra) is a selective aldosterone receptor antagonist mainly used to treat hypertension and heart failure after myocardial infarction, and has the effect of reducing the risk of cardiovascular events. Special attention should be paid to the risk of hyperkalemia when taking medication. Blood potassium levels should be monitored regularly, and the dosage should be adjusted according to individual circumstances.

1. Effect and Function

(1) Hypertension: Eprenone can significantly reduce the occurrence of fatal and non fatal cardiovascular events, especially with good therapeutic effects on stroke and myocardial infarction. Multiple clinical trials have shown that it has a significant effect on reducing hypertension.

(2) Post myocardial infarction heart failure: suitable for improving the survival rate of symptomatic heart failure stable adult patients with reduced ejection fraction after acute myocardial infarction.

2. Medication precautions

(1) Risk of hyperkalemia: especially in patients with renal insufficiency, diabetes or those taking ARB, NSAIDs and other drugs at the same time, the risk is higher. During the treatment period, blood potassium levels should be continuously monitored until the efficacy stabilizes.

(2) Omission treatment: If one dose is missed, there is no need to supplement it. It can be taken at the next regular medication time.

(3) Drug interactions: When used in combination with moderate CYP3A inhibitors, the dosage should be adjusted according to individual pharmacokinetics.

3. Side effects and treatment

(1) Common side effects: Elevated blood cholesterol and triglyceride levels, requiring regular monitoring.

(2) Overtreatment: In cases of severe hypotension or hyperkalemia, symptomatic supportive treatment is necessary, such as fluid replacement, potassium lowering resin, etc. When hemodialysis is ineffective, activated carbon adsorption can be attempted.
