Roflumilast is a phosphodiesterase-4 (PDE4) inhibitor used to reduce the risk of acute exacerbations in patients with chronic obstructive pulmonary disease (COPD). Proper administration and dose adjustment are key to ensuring efficacy and safety.

I. Routine Administration Methods

1. Dosage

The recommended dose is 500 μg once daily.

Take at the same time each day, with or without food.

Swallow the tablet whole; do not chew, split, or crush it.

2. Administration Time

Taking it at a fixed time is recommended to help form a habit.

It may be taken with or without food; food does not affect absorption.

If taken with food, it may reduce some gastrointestinal discomfort.

3. Treatment Course and Evaluation

Continuous use is required to achieve efficacy.

Benefits and tolerability should be evaluated regularly during treatment.

Do not stop the medication on your own; adjust according to medical advice.

II. Missed Dose Management and Dose Adjustment

1. Missed Dose Management

If remembered and there is still a long time until the next dose, take it as soon as possible.

If the next dose is approaching, skip the missed dose.

Do not take a double dose at one time.

After taking the missed dose, continue once daily according to the original schedule.

2. Principles of Dose Adjustment

Generally, no routine dose adjustment is needed.

If intolerance occurs, the doctor may recommend temporarily reducing to 250 μg once daily.

If tolerance improves after reduction, resuming 500 μg may be considered.

Dose adjustment must be performed under medical guidance.

3. Adjustment in Special Populations

Use with caution in patients with hepatic impairment.

Not recommended for patients with moderate or severe hepatic impairment.

Older adults usually do not require dose adjustment.

Low-body-weight patients should be closely monitored for adverse reactions.

III. Precautions and Adverse Reaction Monitoring

1. Common Adverse Reactions

Diarrhea, nausea, headache, insomnia, etc., are relatively common.

Most are mild to moderate and may gradually lessen with treatment.

If symptoms persist or worsen, seek medical attention promptly.

2. Situations Requiring Vigilance

If obvious mood changes, depression, or suicidal thoughts occur, seek medical attention immediately.

Unexplained weight loss should be reported to the doctor.

Arrhythmia or palpitations should be evaluated to determine whether to continue the medication.

3. Medication Reminders

Maintain regular follow-up during medication.

Inform the doctor of all concomitant medications.

Do not increase, decrease, or stop the dose on your own.
