Olmesartan medoxomil lowers blood pressure smoothly, but during treatment attention should be paid to hypotension, hyperkalemia, changes in renal function, and drug interactions to ensure safety and efficacy.

I. Hypotension

High-risk groups: Patients with volume depletion, excessive salt restriction, those taking diuretics, and the elderly.

Manifestations: Dizziness, fatigue, blurred vision, and in severe cases, syncope.

Management:

Start with a low dose and titrate gradually.

Correct dehydration or volume depletion before initiating therapy.

Rise slowly and avoid standing up suddenly.

If symptoms persist, seek medical attention to adjust the dose.

II. Hyperkalemia

Cause: Olmesartan medoxomil can reduce potassium excretion, especially in patients with renal impairment.

High-risk factors: Diabetes, renal impairment, concomitant use of potassium-sparing diuretics or potassium supplements.

Monitoring: Check serum potassium regularly.

Management:

Avoid self-administered potassium supplementation or potassium-containing salt substitutes.

Seek medical attention promptly if muscle weakness or arrhythmia occurs.

Adjust medications or diet as necessary.

III. Renal Function Monitoring

Effects: May cause elevated serum creatinine, especially in patients with bilateral renal artery stenosis.

Monitoring: Check renal function before and during treatment.

Precautions:

Renal impairment is more likely when there is volume depletion or dehydration.

The risk increases with concomitant use of diuretics or nonsteroidal anti-inflammatory drugs.

If creatinine rises significantly, evaluate whether treatment should be continued.

IV. Drug Interactions

Potassium-sparing diuretics: Such as spironolactone, may increase the risk of hyperkalemia.

Nonsteroidal anti-inflammatory drugs: May reduce the antihypertensive effect and impair renal function.

Lithium: Concomitant use may increase serum lithium concentrations; monitoring is required.

Other antihypertensive drugs: May enhance the blood pressure-lowering effect; dose adjustment may be needed.

Diuretics: Especially at high doses, may worsen hypotension and changes in renal function.
