Precautions for taking lenatinib: management of severe diarrhea, hepatotoxicity, and drug interactions
The core of the medication safety of Lenvatinib lies in actively managing three major risks: severe diarrhea, liver injury, and drug interactions. Taking proactive measures can effectively reduce the risk of treatment interruption.
1、 Proactive management of severe diarrhea
Initiate loperamide prophylaxis: start taking loperamide at the first dose, with a first dose of 4mg, followed by an additional 2mg after each bowel movement, with a daily dose not exceeding 16mg, and maintain until several days after the end of treatment.
Fluid replacement and electrolyte balance: Drink fluids containing electrolytes daily. Seek medical attention immediately if you experience thirst, oliguria, or dizziness.
Timing of dose adjustment: If diarrhea still occurs more than 6 times within 24 hours under the maximum dose of antidiarrheal medication, pause lenatinib and restart at a lower dose after recovery.
2、 Monitoring and Response to Hepatotoxicity
Regular liver function testing: In the early stages of treatment, transaminase and bilirubin levels are tested monthly, and the interval can be appropriately extended after stabilization.
Warning symptom identification: When there is yellowing of the skin or sclera, darkening of urine color, pain in the upper right abdomen, or obvious fatigue, the medication should be stopped immediately and liver function should be checked.
Dosage adjustment principle: When there is a grade 3 or above increase in liver enzymes, medication should be suspended and reduced according to the prescribed plan after recovery.
3、 Key drug interaction avoidance
Strong or moderate CYP3A4 inhibitors: Avoid combination therapy (such as ketoconazole and clarithromycin), and if unavoidable, significantly reduce the dose of lenvatinib.
Strong or moderate CYP3A4 inducers: Combination use (such as rifampicin and phenytoin) is prohibited to avoid a decrease in efficacy due to low blood drug concentrations.
Gastric acid regulator: Avoid taking it together with proton pump inhibitors. Antacids or H2 receptor antagonists should be taken at least 3 hours apart from lenatinib.
