Baricitinib: Usage, Dosage, Efficacy, and Precautions
Baricitinib is an oral JAK inhibitor used to treat autoimmune diseases. Standardized medication is the key to ensuring efficacy and safety, and management should be done from three aspects: usage, efficacy monitoring, and risk prevention.
1、 Standard usage and dosage
Conventional dose: The recommended starting dose is once daily, 4mg each time; for specific populations (such as those over 75 years old or those with moderate kidney function impairment), it can be reduced to 2mg per day.
Usage: Can be taken with meals or on an empty stomach. Swallow the whole tablet and do not chew or crush it. It is recommended to take medication at a fixed time every day to maintain stable blood drug concentration.
Omission treatment: If missed, it should be replenished as soon as possible when remembered on the same day; If it is close to the next medication time, skip the missed dose and do not double it.
2、 Key points of efficacy evaluation
Effective time: Clinical improvement usually begins 4-8 weeks after treatment and reaches a relatively stable effect around 12 weeks, requiring patient persistence.
Therapeutic efficacy indicators: Based on specific diseases such as rheumatoid arthritis and alopecia areata, objective indicators such as joint swelling, skin lesion range, or hair regeneration are used to evaluate.
Regular follow-up: Every 3 months, the doctor will comprehensively evaluate the efficacy. If there is not enough response within 16 weeks, the adjustment plan should be considered.
3、 Key precautions
Infection prevention and control: Screening for tuberculosis and hepatitis is necessary before medication; If there are signs of infection such as fever and cough during treatment, seek medical attention promptly.
Thrombosis risk: Those with a history of thrombosis should use it with caution; If there is sudden unilateral limb swelling or chest pain, seek medical attention immediately.
Liver and kidney function monitoring: Regularly test liver enzymes and blood creatinine before and during treatment. If liver enzymes significantly increase, discontinue medication for evaluation.
