Baricitinib is a JAK inhibitor, and when treating inflammatory diseases, special attention should be paid to risks such as infection, thrombosis, and elevated liver enzymes. Scientific management can effectively reduce the occurrence of adverse events.

1、 Management of infection risk

Pre medication screening: TB, viral hepatitis and other screening should be carried out before treatment, and active infections should be postponed.

Be alert to new symptoms: If fever, chills, persistent cough, or sore throat occur during treatment, seek medical attention immediately without delay.

Avoid live vaccines: During medication and for a period of time after discontinuation, live vaccines (such as mumps and chickenpox vaccines) should not be administered.

2、 Prevention of thrombotic events

Assess high-risk factors: For those with a history of deep vein thrombosis, pulmonary embolism, or cardiovascular disease, a strict evaluation of benefits and risks by a doctor is required before use.

Pay attention to early signals: If there is unilateral limb swelling, pain, chest pain, or sudden difficulty breathing, seek emergency medical attention immediately.

Lifestyle adjustments: Avoid prolonged bed rest or sitting for long periods, engage in moderate activity, drink plenty of water, and reduce blood stasis.

3、 Monitoring of elevated liver enzymes

Baseline testing: Liver function (ALT, AST) should be tested before and during the early stages of treatment as a basis for subsequent comparisons.

Regular follow-up: During the treatment period, liver enzymes should be rechecked every 1-3 months according to the doctor's advice. If fatigue, decreased appetite, or jaundice occur, they should be checked at any time.

Dose adjustment: If liver enzymes are significantly elevated (such as exceeding the normal upper limit by 5 times), medication should be suspended and the doctor should decide whether to reduce dosage or change medication after recovery.
