Ritlecitinib: Usage, Dosage, Efficacy, and Precautions
Ritlecitinib is a daily oral JAK inhibitor, and its standardized use requires close attention to three major aspects: standard dosage, efficacy evaluation, and full process safety monitoring. The following are the core points of clinical medication.
1、 Standard usage and dosage
Initial dose: Recommended once a day, 50mg each time, taken whole at a fixed time of the day (such as after breakfast), unaffected by food intake.
Dosage adjustment: If missed, make-up medication can be taken at least 8 hours before the next dose, otherwise skip; Do not double the dosage per day. When using potent CYP3A4 inhibitors (such as ketoconazole) in combination, follow the doctor's advice to reduce the dosage to 30mg/day.
Course management: Continuous medication, no pre-set discontinuation period, and the decision to maintain or discontinue treatment is made by the doctor after efficacy evaluation.
2、 Efficacy monitoring indicators
Target observation window: The key evaluation points are the 4th, 12th, and 24th week of treatment, and symptom scores (such as skin lesion area and severity index) need to be recorded during follow-up visits.
Efficacy evaluation: If satisfactory control is not achieved after 24 weeks (such as improvement<50% compared to baseline), the doctor will reassess whether to switch or combine other treatments.

3、 Core precautions during medication
Infection and laboratory tests: Blood routine and liver function will be checked every 2-4 weeks for the first 3 months before treatment; If there are signs of infection such as fever, sore throat, and frequent urination, take medication temporarily and seek medical attention.
Taboo behavior: live vaccine ban (within 4 weeks of discontinuation); Avoid combining with strong immunosuppressants (such as cyclosporine); Not recommended during lactation.
Special population: Elderly people (≥ 65 years old) do not need to adjust the dosage, but need to strengthen renal function monitoring; Mild to moderate liver injury (Child Pugh A/B) can be used, but severe use is contraindicated.
4、 Notice on Stopping and Changing Medications
