Before using Ritlecitinib, clearly defining contraindications is the first line of defense against serious adverse events. The following two core taboos and related operating instructions must be strictly followed.

1、 Active severe infection: absolutely prohibited

Scope of definition: including but not limited to septicemia, pneumonia, tuberculosis (active phase), invasive fungal infection, herpes zoster (disseminated) or viral infection (such as EB virus active phase).

Operating guidelines: If there is any untreated infection (even if it is a local infection such as cellulitis), the start of treatment should be postponed until the infection is completely controlled and anti infective drugs should be discontinued for at least 2 weeks.

Warning signal: If there is a new onset of high fever (>38.5 ℃), chills, or severe pain in the infected area during treatment, medication should be stopped immediately and medical attention should be sought. Do not continue taking medication.

2、 Unscreened tuberculosis patients: screening must be carried out first

Mandatory screening program: Prior to initiating Ritlecitinib, a tuberculin skin test (TST) or interferon gamma release test (IGRA) must be completed, along with a chest imaging examination (X-ray or CT).

Screening result processing:

Active tuberculosis: Medication is prohibited and standardized anti tuberculosis treatment must be completed by transferring to a specialist.

Latent tuberculosis: Ritlecitinib can only be considered after completing at least one month of prophylactic anti tuberculosis treatment (such as isoniazid) under the guidance of a doctor.

Screening timeliness: The test report must be within 3 months before initiating treatment, and re examination is required if it expires.

3、 Other clearly prohibited situations (supplementary)

Severe liver injury (Child Pugh C) and uncontrolled severe hypertension or diabetes are not recommended.

Pregnant and lactating women (unless the benefits outweigh the risks and require specialized evaluation).

Individuals allergic to any excipients of this product are prohibited from using it.
