Selumetinib: Usage, Dosage, Efficacy, and Precautions

1、 Usage and dosage
1. Standard dose
The recommended dosage for adults is twice a day, 75mg each time (i.e. a total daily dose of 150mg), which can be taken on an empty stomach or with meals.
The interval between two doses of medication is about 12 hours, and it is recommended to take it at a fixed time.
2. Medication method
Swallow the whole capsule, do not chew, crush or open the capsule.
If missed, additional medication can be taken at least 6 hours after the next dose; If it is less than 6 hours, skip it and take it on time next time.
3. Dose adjustment
According to toxicity reactions (such as diarrhea, rash, or liver dysfunction), the dosage can be reduced to twice a day, 50mg or less each time. After the reduction, the original dose cannot be arbitrarily restored.
2、 Efficacy evaluation
1. Imaging examination
Perform CT or MRI scans every 8-12 weeks to evaluate changes in tumor size.
2. Symptom assessment

1. Drug interactions
Avoid co administration with strong CYP3A4 inhibitors such as clarithromycin and itraconazole, otherwise the dosage needs to be adjusted.
Be cautious when using CYP3A4 inducers (such as rifampicin) in combination, as it may reduce efficacy.
2. Laboratory monitoring
Monthly monitoring of creatine kinase (CK) levels, as well as liver and kidney function and blood routine.
3. Special populations
The dosage for children's use should be calculated based on their body surface area; Elderly people do not need adjustment, but monitoring needs to be strengthened.
Prohibited during pregnancy; Women of childbearing age must use effective contraception during medication and within one month after discontinuation.
