Precautions for taking sematinib: cardiomyopathy, eye toxicity (retinopathy), and skin toxicity
During the use of sematinib, it is important to pay close attention to potential toxicity to the myocardium, eyes, and skin. Early identification and intervention are the core of safe medication.
1、 Cardiomyopathy
1. Pre treatment evaluation
Before medication, an echocardiogram must be completed to determine the baseline left ventricular ejection fraction (LVEF) value.
2. Regular monitoring
During the treatment period, LVEF should be rechecked every 3 months; If shortness of breath, fatigue, or lower limb edema occurs, it should be checked in advance.
3. Dose adjustment
When LVEF decreases by more than 10% from baseline and the absolute value is less than 50%, medication should be suspended and reduced before restarting.
2、 Eye toxicity (retinopathy)
1. Baseline ophthalmic examination
Before initiating treatment, a comprehensive ophthalmic assessment should be conducted, including visual acuity, intraocular pressure, and retinal examination.
2. Regular follow-up
During the treatment period, a follow-up ophthalmology examination should be conducted every 3 to 6 months; Seek medical attention immediately when experiencing blurred vision, floaters, or changes in color vision.
3. Indications for discontinuation of medication
If diagnosed with retinal vein occlusion or severe retinal lesions, permanent discontinuation of medication is required.
3、 Skin toxicity
1. Rash and dry skin
Common papules, erythema, or flaking require the use of mild moisturizers to avoid irritating skincare products.
2. Prevention of secondary infections
When severe rash or suppuration occurs, seek medical attention promptly and avoid scratching to prevent secondary bacterial infection.
3. Medication adjustment
If severe (such as extensive exfoliative dermatitis) skin reactions occur, medication should be temporarily suspended and reduced or permanently discontinued after recovery.
