Precautions for using Rabelase: risk of hemolysis, G6PD deficiency, and allergic reactions
When using Fasturtec for the treatment of hyperuricemia in tumor lysis syndrome, special attention should be paid to the risk of hemolysis, contraindications for G6PD deficiency, and allergic reactions. The following provides a detailed explanation of precautions from these three aspects to ensure medication safety.
1. Hemolysis risk and G6PD deficiency
(1) Labridase may induce hemolytic anemia, especially in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency, which poses a high risk. Before medication, G6PD activity must be tested and confirmed to be normal before use.
(2) Patients with G6PD deficiency are prohibited from using Rabelase because the hydrogen peroxide produced by drug metabolism can cause damage to the red blood cell membrane, leading to acute hemolysis and even renal failure.
2 Allergic reactions
(1) About 5% of patients may experience allergic reactions, manifested as rash, fever, bronchospasm, and even anaphylactic shock. The first administration should be carried out in a medical facility with emergency conditions.
(2) It is contraindicated for individuals who are allergic to uric acid oxidase or excipients (such as mannitol). If an allergic reaction occurs, the medication should be stopped immediately and antihistamines or adrenaline treatment should be given.
3 Other related monitoring
(1) During medication, daily monitoring of hemoglobin and reticulocyte counts is necessary to be alert to early signs of hemolysis. If hemoglobinuria or jaundice occurs, the medication should be stopped immediately.
(2) Allergic reactions often occur within 2 hours after administration. It is recommended to closely observe for at least 1 hour after administration and prepare emergency equipment.
