As a commonly used lipid-lowering medication, the use of rosuvastatin calcium tablets requires special attention to liver function monitoring, muscle pain identification, and drug interaction management. The following is an explanation of key precautions to help with safe medication.

1. Key points for liver function monitoring

(1) Baseline examination: ALT/AST must be tested before medication, and values exceeding 3 times ULN are prohibited;

(2) Regular follow-up: First follow-up after 4 weeks of medication, followed by monitoring every 3-6 months thereafter;

(3) Exception handling: ALT/AST should be discontinued if it continues to exceed 3 times the ULN, and permanently disabled if it exceeds 5 times the ULN.

2. Muscle pain identification and management

(1) Typical manifestations: Symmetrical proximal muscle weakness with elevated CK;

(2) Hazard threshold: Stop medication immediately if CK>10 times ULN or accompanied by brown urine;

(3) High risk factors: Elderly, renal insufficiency, and concurrent use of beta drugs require a reduction in dosage.

3. Important drug interactions

(1) Cyclosporine: Combination can increase the AUC of rosuvastatin by 7 times, absolutely contraindicated;

(2) Jifenozil: When used in combination, the dosage of statins should not exceed 10mg/day;

(3) Antacids: should be taken every 2 hours, otherwise their bioavailability will be reduced by 50%.

4. Precautions for special populations

(1) Asian patients: The starting dose should be 5mg, with a maximum recommended dose of 20mg/day;

(2) Renal insufficiency: patients with GFR<30ml/min start at 5mg;

(3) Hypothyroidism: It is necessary to correct thyroid function before starting treatment.

5. Other safety warnings

(1) Grapefruit: Daily intake>1.2L may increase the risk of adverse reactions;

(2) Alcohol: Excessive drinking increases the risk of liver damage;

(3) Blood glucose monitoring: may increase HbA1c by 0.3% -0.5%.
