1. Contraindications

(1) It is contraindicated for individuals who are allergic to bedaquiline or its excipients, as it may cause serious allergic reactions such as rash, difficulty breathing, etc.

(2) It is contraindicated for patients with severe liver function impairment, as impaired drug metabolism may lead to toxic accumulation.

(3) Patients with a history of QT interval prolongation or arrhythmia should avoid using bedaquiline, as it may exacerbate cardiac electrophysiological abnormalities.

(4) When using potent CYP3A4 inhibitors (such as ketoconazole) or inducers (such as rifampicin) in combination, it is contraindicated, which may cause abnormal fluctuations in blood drug concentration.

2. Medication for special populations

(1) Pregnant women

Animal studies have shown that Bedazoline has embryotoxicity. Pregnant women can only use it when there is drug resistant tuberculosis and no alternative plan, so the risk benefit ratio needs to be strictly evaluated.

(2) Lactating women

It is not clear whether the medication is secreted through breast milk. It is recommended to suspend breastfeeding during treatment to avoid potential adverse reactions in infants.

(3) Child patients

The safety and efficacy of patients under 18 years old have not been established, and it is not recommended to use it unless in clinical trials or under special approval.

(4) Elderly patients

Patients over 65 years old should use medication with caution, as liver and kidney function may decrease, and electrocardiogram and liver function monitoring should be strengthened.

(5) Liver function impairment

Patients with mild to moderate liver function impairment should reduce their use and closely monitor liver function indicators; Severely impaired individuals are prohibited from using it.

(6) Renal dysfunction

Patients with mild to moderate renal impairment do not need to adjust the dosage, but adverse reactions need to be monitored; The data of severely injured individuals is limited, and it is recommended to use it with caution.

(7) Population with fertility potential

Women of childbearing age should take effective contraceptive measures during medication and within 3 months after discontinuation, and male patients should also pay attention to contraception.
