Regorafenib is a multi kinase inhibitor mainly used for the treatment of specific advanced cancers. When using it, attention should be paid to cardiovascular and liver function, as well as medication safety for special populations.

1 Indications

(1) Colorectal cancer

Suitable for patients with metastatic colorectal cancer who have previously received treatment with fluorouracil, oxaliplatin, and irinotecan.

(2) Gastrointestinal stromal tumor

Used for locally advanced, unresectable, or metastatic gastrointestinal stromal tumors previously treated with imatinib and sunitinib.

(3) Hepatocellular carcinoma

Approved for hepatocellular carcinoma patients who have failed sorafenib treatment and require Child Pugh A liver function.

2 Medication precautions

(1) Cardiovascular monitoring

During the treatment period, blood pressure needs to be monitored weekly. If grade 3 hypertension occurs, medication should be suspended, and grade 4 should be permanently discontinued.

(2) Liver function management

ALT/AST testing should be conducted before treatment and every 2 weeks. If liver enzymes>5 × ULN or bilirubin>3 × ULN occur, the medication should be stopped immediately.

(3) Wound healing

Discontinue medication at least 2 weeks before major surgery and resume use only after complete wound healing after surgery.

(4) Reproductive toxicity

Patients of childbearing age should take efficient contraceptive measures and avoid pregnancy during treatment and within 2 months after discontinuation of medication.

(5) Drug interactions

Avoid using strong CYP3A4 inducers or inhibitors in combination, as it may significantly affect drug concentration.

3 Special precautions for storage

(1) Original packaging preservation

The original packaging box and sealed bottle of the medicine must be retained, and the bottle cap should be tightened immediately after taking to reduce contact with air and moisture.

(2) Child safety

The storage location should be kept out of reach of children. It is recommended to lock or place it at a high place to prevent accidental ingestion.

(3) Exception handling

If the pills change color, shatter, or the packaging is damaged, they should be immediately stopped from use and a pharmacist or medical institution should be contacted.
