The contraindications, foods, and restrictions of Regorafenib must be strictly followed to ensure the safety and effectiveness of the medication. The following provides a detailed explanation from three aspects: taboo groups, food interactions, and usage restrictions.

1 Taboo population

(1) Pregnant women

Regorafenib has embryonic fetal toxicity and is contraindicated during pregnancy. Women of childbearing age should take effective contraceptive measures during medication and within 2 months after discontinuation.

(2) Severe liver dysfunction

It is contraindicated for patients with Child Pugh C grade liver dysfunction as drug metabolism may be significantly affected, increasing the risk of toxicity.

(3) Active bleeding or wound complications

Individuals with active bleeding, recent surgery, or unresolved wounds are prohibited from using the medication as it may inhibit platelet function and wound healing.

2 Forbidden Foods

(1) Grapefruit and its products

It is prohibited to take it with grapefruit, as the furan coumarin it contains can inhibit CYP3A4 enzyme, leading to an abnormal increase in the blood concentration of regorafenib.

(2) High-fat diet

Avoid taking with high-fat meals (fat content>60%) as it may increase medication absorption variability. It is recommended to take after low-fat meals.

3 Usage Restrictions

(1) Cardiovascular monitoring

Regular monitoring of blood pressure and electrocardiogram is necessary during medication, as it may cause hypertension or QT interval prolongation, especially in elderly patients.

(2) Liver function monitoring

Liver function should be tested before treatment and every 2 weeks. If AST/ALT>3 × ULN or bilirubin elevation occurs, medication should be suspended.

(3) Stop medication before surgery

Discontinue medication at least 2 weeks before elective surgery to reduce the risk of bleeding; Medication can only be resumed after the wound has fully healed after surgery.
