Indications and medication precautions for Ziska Deucravatinib in Bangladesh
Deucravatinib is a novel oral TYK2 inhibitor primarily used for the treatment of moderate to severe plaque psoriasis and psoriatic arthritis. Special attention should be paid to infection risk, laboratory indicator monitoring, and medication adjustment for special populations when taking medication.
1. Indications
(1) Plaque psoriasis: suitable for adult moderate to severe patients with insufficient or intolerant response to cyclosporine, methotrexate, or phototherapy.
(2) Psoriatic arthritis: can improve joint swelling, tenderness, and skin lesions, and should be used in combination with non pharmacological therapies.
2. Medication precautions
(1) Infection risk: Screening for tuberculosis is required before treatment, and it is prohibited during active infection. Assess immediately when fever or other symptoms of infection appear.
(2) Laboratory monitoring: Regularly check lymphocyte count, liver enzymes, and creatine phosphokinase, especially every 3 months during the initial stage of treatment.
(3) Vaccination: Avoid using live vaccines, as inactivated vaccines may be administered during periods of weakened efficacy.
3. Medication for special populations
(1) Liver dysfunction: Mild damage does not require dose adjustment, moderate to severe patients should use with caution and strengthen monitoring.
(2) Pregnancy and lactation: Animals show embryotoxicity, and women of childbearing age need to take effective contraception until 4 weeks after discontinuation.
(3) Elderly patients: People aged 65 and above need to be evaluated for liver and kidney function, and the starting dose may need to be reduced.
4. Adverse reaction management
(1) Upper respiratory tract infection: The most common adverse reaction is usually mild, and symptomatic treatment may be necessary if necessary.
(2) Headache: It often occurs in the early stages of treatment and lasts for more than one week, so dosage adjustment should be considered.
(3) Elevated creatine kinase: If it exceeds the upper limit of 5 times the normal value, medication should be suspended and the cause investigated.
