Prohibited populations, foods, and restrictions of Roflumilast
Roflumilast is a selective phosphodiesterase-4 inhibitor primarily used for the treatment of chronic obstructive pulmonary disease (COPD). There are specific contraindications, dietary restrictions, and medication conditions for its use, which must be strictly followed to ensure medication safety. The following is a detailed explanation of relevant taboos and restrictions.
1. Taboo population
(1) Pregnancy and lactation period
Animal studies have shown embryotoxicity, and pregnant women should avoid using it. Medications may be secreted through breast milk, and lactating women should discontinue medication or suspend breastfeeding.
(2) Children and Adolescents
The safety and efficacy of using it for patients under 18 years old have not been established, and it is not recommended.
(3) Severe liver dysfunction
Child Pugh Class C patients are contraindicated, and patients with moderate to severe liver damage should be carefully assessed for risk.
(4) History of mental illness
Individuals with a history of depression or suicidal tendencies are prohibited from using it, as it may worsen mental symptoms.
2. Taboo foods and drugs
(1) Grapefruit and its products
May inhibit CYP3A4 metabolic enzymes, increase blood drug concentration and risk of adverse reactions, and should be avoided during medication.
(2) Strong CYP3A4 inducer
Rifampicin, carbamazepine, and other drugs can reduce the efficacy of roflunomide, and should be avoided in combination or adjusted in dosage.
(3) Theophylline drugs
Combination use may increase the incidence of adverse reactions, and it is necessary to monitor the blood concentration of theophylline.
3. Medication restrictions
(1) Liver function monitoring
Regular liver function testing is required before treatment and within 6 months of treatment. If ALT/AST levels increase by more than 3 times ULN, medication should be discontinued.
(2) Mental symptom monitoring
During medication, it is necessary to closely monitor emotional changes. If depression or suicidal thoughts occur, the medication should be stopped immediately and medical attention should be sought.
(3) Weight management
May lead to weight loss. Patients with BMI<18.5kg/m ² should use caution and regularly monitor their nutritional status.
(4) Acute exacerbation of COPD
Not suitable for the relief treatment of acute bronchospasm, it should be used in conjunction with short acting bronchodilators.
4. Precautions for special populations
(1) Elderly patients
Patients aged 65 and above do not need to adjust the dosage, but need to strengthen adverse reaction monitoring.
(2) Mild liver and kidney dysfunction
Patients with mild to moderate liver damage or any degree of kidney damage do not need to adjust the dosage, but the monitoring period needs to be extended.
