The usage and dosage of Nebivolol need to be adjusted according to individual indications. 1. Usage and dosage

(1) Hypertension treatment

The initial dose for adults is 5mg once daily, which can gradually increase to a maximum of 40mg/day. Suggested to take in the morning, unrelated to food. When blood pressure control is poor, other antihypertensive drugs can be used in combination.

(2) Chronic heart failure

Starting from 1.25mg once daily, the dose should be doubled every 2 weeks to reach the target dose of 10mg/day. During the treatment period, continuous monitoring of cardiac function is necessary. If hypotension or bradycardia occurs, the increment should be paused.

2. Medication for special populations

(1) Liver dysfunction

Child Pugh B/C patients are prohibited from using; The starting dose for mildly impaired individuals should be halved to 2.5mg/day, and the titration rate should be slowed down. The drug exposure of patients with severe liver damage can increase by 5 times.

(2) Renal insufficiency

EGFR ≥ 30mL/min does not require adjustment; EGFR<30mL/min, maximum dose 10mg/day. Hemodialysis patients are prohibited from using the medication as it cannot be cleared through dialysis.

(3) Elderly patients

Individuals aged 65 and above should start with the lowest dose (2.5mg/day for hypertension and 0.625mg/day for heart failure), with incremental intervals extended to 4 weeks. We need to strengthen the monitoring of fall risk.

(4) Pregnancy period

May cause fetal bradycardia, hypoglycemia, and intrauterine growth restriction. Use only when the benefits clearly outweigh the risks, and discontinue medication 48 hours before delivery.

(5) Lactation period

Medications can be secreted into breast milk, and feeding or medication should be suspended during lactation. If medication is necessary, monitor the baby for low blood pressure or bradycardia.

(6) Children's medication

The safety and efficacy of patients under 18 years old have not been established and are not recommended for use. It is recommended to prioritize the use of other beta blockers for adolescent hypertension.
