Indications and Medication Precautions for Alectinib in Laos
Alectinib is a second-generation ALK inhibitor mainly used for the treatment of ALK positive non-small cell lung cancer, with significant efficacy but special medication risks should be noted. The following provides a detailed explanation from two aspects: indications and medication precautions.
1. Indications
(1) Frontline treatment
For first-line treatment of adult patients with ALK positive metastatic non-small cell lung cancer, the median progression free survival can reach 34.8 months, significantly better than crizotinib.
(2) Second line treatment
Used for ALK positive metastatic non-small cell lung cancer after failed treatment with crizotinib, with an objective response rate of approximately 44% and a median response duration of 11.2 months.
2. Medication precautions
(1) Hepatotoxicity monitoring
Monitor ALT/AST/bilirubin levels every 2 weeks for the first 3 months of treatment, followed by regular follow-up. If the transaminase level increases by more than 5 times, medication should be suspended and reduced or stopped after recovery.
(2) Interstitial lung disease
Be alert to new cases of respiratory distress or worsening cough. Suspected cases should immediately discontinue medication and receive glucocorticoid treatment.
(3) Bradycardia
May cause symptomatic bradycardia, regularly monitor heart rate and blood pressure during medication, and avoid combination with antihypertensive drugs.
(4) Elevated creatine phosphokinase
About 43% of patients experience grade 3-4 CPK elevation and need to monitor muscle pain symptoms, and suspend medication if necessary.
(5) Photosensitive sensitivity
About 10% of patients experience photosensitive reactions. It is recommended to avoid direct sunlight during medication and use sunscreen measures.
(6) Embryotoxicity
Women of childbearing age should take effective contraceptive measures during treatment and one week after discontinuing medication, and should not use them during lactation.
