Deucravatinib requires special attention to infection risk, cardiovascular events, and medication management for special populations. During treatment, regular monitoring of blood routine, liver function, and inflammatory indicators is necessary. Women of childbearing age should strictly use contraception.

1. Risk of infection

(1) It may cause bacterial, viral or fungal infections, including tuberculosis recurrence or hepatitis B reactivation.

(2) TB and hepatitis B virus carrier status should be screened before use, and active infection is prohibited.

2. Cardiovascular events

(1) May increase the risk of myocardial infarction and stroke, especially in patients with concomitant cardiovascular risk factors.

(2) Monitor blood pressure and blood lipids during treatment, and assess immediately if chest pain or neurological symptoms occur.

3. Laboratory monitoring

(1) Check blood routine every 4 weeks, focusing on neutropenia or lymphopenia.

(2) Monitor liver function monthly and suspend medication if ALT/AST>3 times the normal value.

4. Medication for special populations

(1) Pregnancy/lactation period

Prohibited during pregnancy (animals showing embryotoxicity), discontinued during lactation or feeding.

(2) Liver dysfunction

Child Pugh B/C patients need to reduce their dosage by 50% and monitor their liver function weekly.

(3) Elderly patients

Individuals aged 65 and above have an increased risk of infection, but there is no need to adjust the dosage.

5. Other precautions

(1) Avoid combining with potent CYP3A4 inhibitors as it may increase blood drug concentration.

(2) Vaccination with live vaccines requires discontinuation of medication for at least 4 weeks.
