The contraindications for Deucravatinib include active infections, severe liver dysfunction, etc. Special populations need to adjust their medication regimen based on pregnancy status, age, and liver and kidney function. Strict contraception is required during the reproductive age period.

1. Contraindications

(1) Active tuberculosis or hepatitis B infection: screening is required before medication, and active infection is prohibited.

(2) Severe liver dysfunction: Child Pugh Class C patients are prohibited from using it, while Class B patients require a 50% reduction in dosage.

(3) Allergic reactions: Do not use for those who are allergic to drug ingredients.

(4) Contraindications for combination therapy: Avoid combination therapy with potent CYP3A4 inhibitors.

2. Medication for special populations

(1) Pregnancy period

Based on animal embryonic toxicity data, pregnant women are prohibited from using medication, and the pregnancy status needs to be confirmed before use.

(2) Lactation period

Medications may be secreted through breast milk, and breastfeeding or medication should be stopped during treatment.

(3) Childbearing age period

Women need to take effective contraceptive measures during treatment and for at least 4 weeks after discontinuing medication.

(4) Elderly patients

Patients aged 65 and above have an increased risk of infection, but there is no need to adjust the dosage and monitoring needs to be strengthened.

(5) Liver dysfunction

Reduce dosage by 50% for Child Pugh B patients, discontinue use for C patients, and monitor liver function indicators weekly.

(6) Renal insufficiency

Mild to moderate damage does not require dose adjustment, and caution is needed for patients with end-stage renal disease due to limited data.

(7) Children's medication

The safety and efficacy of using it for patients under 18 years old have not been established and is not recommended.
