Eltrombopag is an oral thrombopoietin receptor agonist primarily used to treat thrombocytopenia by stimulating megakaryocyte proliferation and differentiation to increase platelet count. It is suitable for patients with specific chronic liver diseases or immune thrombocytopenia.

1. Core mechanism of action

(1) Target binding: Selective binding and activation of thrombopoietin receptor (TPO-R), simulating the action of endogenous thrombopoietin.

(2) Cellular effect: Promotes proliferation, differentiation, and maturation of megakaryocytes in the bone marrow, thereby increasing platelet production.

2 Main clinical efficacy

(1) Chronic liver disease-related thrombocytopenia: Elevate platelets in adult patients who plan to undergo surgery to safe levels and reduce the risk of bleeding.

(2) Chronic immune thrombocytopenia (ITP): Used to maintain platelet counts in adult and pediatric patients who are unresponsive to cortisol/immunoglobulin therapy.

3 Pharmacodynamic characteristics

(1) Effective time: After oral administration, platelet count usually starts to rise within 4-7 days and reaches its peak within 12-16 days.

(2) Dose correlation: The therapeutic effect is positively correlated with the dosage administered, and the dose needs to be adjusted according to platelet count.

4 Special precautions

(1) Liver function impact: Patients with severe liver injury need to adjust the dosage, and Child Pugh C-grade patients have a 20% -30% reduction in exposure.

(2) Drug interaction: When taking with multivalent cationic drugs, an interval of 4 hours is required to avoid absorption interference.
