Cabozantinib is a multi-target tyrosine kinase inhibitor that has been approved by the FDA for the treatment of various malignant tumors, including medullary thyroid cancer, radiation iodine refractory thyroid cancer, renal cell carcinoma, and hepatocellular carcinoma. Its indications cover adult and pediatric patients over 12 years old.

1. Indications for thyroid cancer

(1) Medullary thyroid carcinoma: Used as first-line treatment for progressive and metastatic medullary thyroid carcinoma, approved based on data on progression free survival improvement, capsule formulations are specifically designed for this indication.

(2) Differentiated thyroid cancer: Tablets are suitable for locally advanced or metastatic diseases in patients aged 12 years and above who are refractory to radioactive iodine therapy, and should be used after failure of VEGFR targeted therapy.

2. Urinary system tumors

(1) Renal cell carcinoma: monotherapy is used for advanced treatment, and combination with nivolumab can be used as a first-line regimen. The daily dose is adjusted according to the combination therapy.

3. Digestive system tumors

(1) Hepatocellular carcinoma: Used for second-line treatment after failed sorafenib treatment, administered daily in 60mg tablets, and approved as an orphan drug by the FDA.

4. Medication for special populations

(1) Children's medication: 60mg per day for those aged 12 and above with a body surface area ≥ 1.2 square meters, 40mg for those<1.2 square meters, and continue until disease progression.

(2) Formulation difference: Capsules (for medullary thyroid cancer) and tablets (for other indications) are not interchangeable.
