The use of Alpelisib should focus on high blood sugar, skin reactions, and hypersensitivity risks, and regular monitoring of blood sugar levels and emergency response plans should be prepared. Medication should be taken at a fixed time with meals, and can be supplemented within 9 hours of missed intake. Vomiting does not require supplementation. 1. Basic Medication Standards

(1) Dosage and Administration: It is recommended to take 300mg (2 tablets 150mg) orally once a day, with meals, and swallow the whole tablet. Do not chew or break it open. Continue medication until disease progression or intolerable toxicity occurs.

(2) Omission treatment: Supplement can be taken within 9 hours (with meals), and skip the daily dose if it exceeds 9 hours. After vomiting, there is no need to take additional medication. The next day, the medication will be administered according to the original plan.

2. Serious risks that require urgent handling

(1) Hypersensitivity reaction: may cause anaphylactic shock (with an incidence rate of 0.7%), manifested as difficulty breathing, rash, etc. Permanent discontinuation of medication and emergency treatment are required.

(2) Severe skin reactions, including Stevens Johnson syndrome, require temporary suspension of medication and dermatological consultation upon symptom onset, and permanent discontinuation of medication upon diagnosis.

(3) Pneumonia: incidence rate of 1.8%, new respiratory symptoms require immediate discontinuation of medication for evaluation, and permanent discontinuation after diagnosis.

3. Long term monitoring and management

(1) High blood sugar: Fasting blood sugar and HbA1c are measured before treatment, with weekly monitoring for the first 2 weeks and once every 4 weeks thereafter. Medication should be temporarily suspended for level 3 hyperglycemia and permanently discontinued for level 4 hyperglycemia.

(2) Diarrhea/colitis: Symptomatic treatment for grade 1, medication should be temporarily suspended for grades ≥ 2, and permanently discontinued for grades 4. Pay attention to fluid replacement and the use of glucocorticoids.

4. Precautions for special populations

(1) Childbearing age population: Strict contraception is required during treatment and one week after discontinuation, and pregnant women are prohibited from using it. Male patients need to use condoms.

(2) Patients with diabetes: type 1 or uncontrolled type 2 are prohibited. Those with good control need to strengthen glucose lowering monitoring. Metformin pre-treatment may be considered.

5. Principle of dose adjustment

(1) Initial reduction: from 300mg to 250mg, with a minimum of 200mg/day. Only one dose reduction is allowed for pancreatitis.

(2) Adjustment of high blood sugar: pause or reduce according to the grading, and administer the next low dose after recovery.
