Precautions for taking Pemabet: Elevated liver enzymes, muscle pain, and drug interactions
Pemafibrate is used for hypertriglyceridemia. During treatment, it is important to focus on monitoring liver- and muscle-related adverse reactions and to remain alert for drug interactions.
Elevated Liver Enzymes
Liver function, including baseline ALT, AST, bilirubin, etc., should be tested before treatment.
Liver function should be rechecked regularly during treatment, especially after starting the drug and after dose adjustments.
If liver enzymes are mildly elevated, continued observation and more frequent monitoring may be carried out according to medical advice; if markedly elevated or accompanied by jaundice, fatigue, loss of appetite, or dark urine, the drug should be stopped and medical attention sought.
Liver function abnormalities must not be ignored just because blood lipids have decreased, and the dose must not be increased on one's own.
Muscle Pain and Muscle-Related Risks
During treatment, if unexplained muscle pain, muscle weakness, tenderness, or dark urine occurs, inform the doctor promptly.
The doctor may test creatine kinase (CK) and reduce or stop the drug if necessary.
In patients with renal impairment, hypothyroidism, or concomitant use of statins, the risk of myopathy may be increased, and monitoring should be strengthened.
Avoid adding other lipid-lowering drugs or supplements on one's own, especially products that may affect muscle metabolism.
Drug Interactions
When Pemafibrate is used together with other lipid-lowering drugs (such as statins), attention should be paid to muscle pain and changes in liver enzymes, and the dose should be adjusted if necessary.
Concomitant use with anticoagulants may affect coagulation parameters, and monitoring should be carried out according to medical advice.
Avoid combination with fibrates such as gemfibrozil to prevent increasing muscle and liver adverse reactions.
When using drugs metabolized by the liver, inform the doctor of the full medication list, including prescription drugs, over-the-counter drugs, and herbal medicines.
Avoid heavy alcohol consumption during treatment to prevent increasing the burden on the liver.
Other Reminders
The drug may be taken with meals or on an empty stomach, but should be taken at a fixed time; after a missed dose, do not double the dose.
Monitor blood lipids, liver function, and CK regularly, and return for follow-up promptly if abnormalities occur.
Any dose adjustment and discontinuation should be decided by the doctor.
