Lascufloxacin is a quinolone antibacterial agent used for respiratory tract and skin infections. Its absorption is only limitedly affected by food, but the timing of administration, management of missed doses, and dosage adjustment still need to be properly mastered. The following explains from three aspects.

I. Fasting or with meals

1. Either before or after meals

Lascufloxacin taken with food or on an empty stomach shows no significant difference in absorption extent, and is generally unaffected by meals.

2. Fixed time recommended

To aid memory and maintain stable blood drug concentrations, it is recommended to take it at a fixed time daily, either before or after meals.

3. Interval from metal ion drugs

If taking antacids containing aluminum or magnesium, or metal cation drugs such as iron, calcium, or zinc supplements concomitantly, they should be separated by at least 2 hours to avoid affecting absorption.

4. Take with sufficient water

Take with sufficient plain water; avoid taking the medicine with calcium-containing drinks such as milk or soy milk, and avoid taking it with caffeine-containing beverages.

II. Management of missed doses

1. Take as soon as remembered

If a dose is missed and there is still much time before the next dose, take the missed dose as soon as possible.

2. Skip if close to the next dose

If the next dose time is approaching, skip the missed dose and take the next dose as originally scheduled.

3. Do not double the dose

Do not take double the dose at one time to make up for a missed dose, as this may increase the risk of adverse reactions.

4. Do not interrupt the course arbitrarily

After a missed dose, the entire course should still be completed as prescribed to avoid reduced efficacy or drug resistance caused by intermittent medication.

III. Dosage adjustment

1. Adjust according to renal function

This product is mainly excreted by the kidneys. Patients with reduced renal function need dosage adjustment based on creatinine clearance, and those with moderate to severe renal impairment usually require dose reduction.

2. Hepatic impairment

Mild to moderate hepatic impairment generally requires no adjustment; severe hepatic impairment should be used with caution and closely observed.

3. Elderly patients

Renal function often declines in elderly patients. Renal function should be assessed before administration, with dose reduction if necessary, and attention should be paid to central nervous system and tendon adverse reactions.

4. Match course with indication

Dosage and course should be determined based on the infection site, severity, and pathogenic findings, and should not be increased, decreased, or prolonged arbitrarily.

5. Management of adverse reactions

If tendon pain, peripheral neuropathy symptoms, central nervous system reactions, or severe rash occur, the drug should be stopped and medical attention sought, with the physician deciding whether to adjust the regimen.
