Contraindications for esomeprazole: Notice for patients allergic to PPI and severe liver dysfunction
Although esomeprazole is widely used, some populations require contraindication or cautious use, especially those allergic to proton pump inhibitors (PPIs) and patients with severe hepatic impairment.
I. Those allergic to PPIs
1. Scope of contraindication
Contraindicated in patients allergic to esomeprazole, other PPIs (such as omeprazole, lansoprazole, pantoprazole, rabeprazole), or any excipient of this product.
2. Allergic manifestations
Rash, pruritus, urticaria, facial or laryngeal edema, dyspnea, etc. may occur; severe cases may develop anaphylactic shock.
3. Management principles
A detailed allergy history should be obtained before medication; once allergic symptoms occur, the drug should be stopped immediately and medical care sought; severe allergic reactions require emergency management, and similar drugs should be avoided thereafter.
4. Note on cross-allergy
Cross-allergy may exist among different PPIs; those who have been allergic to any PPI should consult a physician before using other PPIs.
II. Patients with severe hepatic impairment
1. Cautious use and dose adjustment
In severe hepatic impairment, drug metabolism is slowed and plasma concentration may increase; use with caution, and reduce the dose or extend the dosing interval under medical guidance if necessary.
2. Monitor liver function
Liver function should be monitored regularly during medication; if fatigue, poor appetite, jaundice, dark urine, etc. occur, seek medical care promptly.
3. Mild to moderate hepatic impairment
Patients with mild to moderate hepatic impairment usually do not require dose adjustment, but should still use the drug as prescribed and observe for adverse reactions.
4. Avoid long-term high-dose use
Patients with severe hepatic impairment should avoid long-term high-dose use to reduce the risk of drug accumulation and adverse reactions.
III. Other populations requiring attention
1. Long-term users
Long-term use of PPIs may increase the risk of fracture, hypomagnesemia, intestinal infection, etc.; the necessity of medication should be assessed regularly.
2. Patients on concomitant medication
Concomitant use with drugs such as clopidogrel may affect efficacy; consult a physician or pharmacist.
3. Pregnancy and lactation
Medication during pregnancy and lactation should weigh the benefits and risks and be used under medical guidance.
