Bedaquiline is an important drug in combination therapy for multidrug-resistant tuberculosis, but its cardiac and hepatic toxicity and drug interactions require special attention, and strict monitoring should be performed during treatment.

I. QT Interval Prolongation

1. Risk mechanism

Bedaquiline can prolong ventricular repolarization, leading to QT interval prolongation and increasing the risk of serious arrhythmias such as torsades de pointes.

2. Monitoring requirements

Electrocardiography should be performed regularly before and during treatment, especially in the first few weeks and when used concomitantly with other drugs that prolong the QT interval.

3. Avoid concomitant use

Avoid concomitant use with other drugs known to prolong the QT interval, such as certain antiarrhythmic drugs, quinolones, macrolides, and antipsychotics.

4. Electrolyte management

Maintain serum potassium and magnesium within normal ranges. Hypokalemia and hypomagnesemia increase the risk of arrhythmia and should be corrected promptly.

5. High-risk populations

Patients with a history of heart disease, arrhythmia, electrolyte disorders, or thyroid dysfunction should be used with caution and monitored more closely.

6. Management principles

If the QT interval is significantly prolonged or arrhythmia occurs, discontinue the drug and consult a cardiologist, and adjust the combination regimen if necessary.

II. Hepatotoxicity

1. Risk manifestations

Bedaquiline can cause elevated liver enzymes, and a small number of patients may develop severe liver injury.

2. Monitoring requirements

Liver function should be tested regularly before and during treatment, including transaminases, bilirubin, and alkaline phosphatase.

3. Clinical symptoms

If fatigue, loss of appetite, nausea, vomiting, abdominal pain, jaundice, or dark urine occurs, seek medical attention immediately.

4. Management principles

If drug-induced liver injury is suspected, suspend the drug and evaluate. If it is confirmed to be related to bedaquiline and severe, permanently discontinue the drug.

5. High-risk populations

Patients with pre-existing liver disease, alcohol abuse, or concomitant use of other hepatotoxic drugs are at increased risk and require closer monitoring.

III. Drug Interactions

1. Metabolic pathway

Bedaquiline is mainly metabolized by CYP3A4, and its blood concentration is susceptible to CYP3A4 inducers or inhibitors.

2. Strong inducers

Rifampin, rifapentine, carbamazepine, phenytoin, and others can significantly reduce bedaquiline concentrations and should be avoided.

3. Strong inhibitors

Ketoconazole, itraconazole, clarithromycin, and others can increase bedaquiline concentrations and increase the risk of toxicity, so they should be used with caution and monitored.

4. QT-prolonging drugs

When used concomitantly with other QT-prolonging drugs, the risk of arrhythmia is additive, and such combinations should be avoided as much as possible or electrocardiographic monitoring should be strengthened.

5. Antituberculosis combination regimens

Bedaquiline must be used in combination with other antituberculosis drugs, and drug interactions and tolerability should be comprehensively considered when formulating the regimen.

6. Medication disclosure

Patients should inform their doctor of all medications they are taking, including prescription drugs, over-the-counter drugs, and herbal medicines, and should avoid adding or stopping drugs on their own.
