Roflumilast: Oral and Topical Dosage and Administration
Roflumilast is currently available in two dosage forms, oral and topical. Their indications, usage, and dosage differ, and they must be used correctly according to the specific dosage form and treatment goal.
I. Usage and Dosage of the Oral Dosage Form
1. Indications
Used to reduce the risk of acute exacerbations in patients with chronic obstructive pulmonary disease (COPD).
As add-on treatment to bronchodilator therapy.
2. Recommended dose
500 μg once daily.
May be taken with food or on an empty stomach.
Swallow the tablet whole; do not chew, split, or crush it.
3. Dose adjustment
If intolerance occurs, the doctor may recommend temporarily reducing to 250 μg once daily.
If tolerance improves, resuming 500 μg may be considered.
Adjustment must be made under medical guidance; do not increase or decrease the dose on your own.
II. Usage and Dosage of the Topical Dosage Form
1. Indications
Used to treat skin diseases such as plaque psoriasis.
Suitable for adults and pediatric patients in certain age groups.
2. Recommended usage
Apply a thin layer to the affected area once daily.
Gently rub in until fully absorbed; avoid applying over a large area.
Wash hands after use, unless the hands are the treatment area.
3. Precautions
For external use only; do not take orally.
Avoid contact with eyes, mouth, and other mucous membranes.
Do not use on broken skin or infected areas.
III. Dosage Form Selection and Medication Reminders
1. Distinguishing dosage forms
The oral dosage form is used for COPD, and the topical dosage form is used for skin diseases.
The two dosage forms must not be used interchangeably.
The correct dosage form should be selected according to the doctor's prescription.
2. Management of missed doses
Missed oral dose: take it as soon as possible when remembered; skip it if the next dose is approaching.
Missed topical dose: apply it as soon as possible when remembered, and continue according to the original schedule.
Do not use a double dose at one time.
3. Special populations
The oral dosage form is not recommended for patients with moderate or greater hepatic impairment.
Patients with a history of depression require evaluation before using the oral dosage form.
The topical dosage form should be used according to the instructions and medical advice.
4. Medication reminders
Use strictly according to medical advice; do not stop the medication or change the dosage form on your own.
