During long-term use of sirolimus tablets, it is important to focus on preventing adverse reactions such as hyperlipidemia, proteinuria, interstitial pneumonia, and delayed wound healing. Scientific monitoring and timely intervention are key to ensuring medication safety.

1. Management of hyperlipidemia

Regular monitoring: Blood lipids (total cholesterol, triglycerides, LDL-C) are measured every 3-6 months before medication and after treatment.

Lifestyle intervention: Adopt a low-fat, low cholesterol diet, increase dietary fiber, and adhere to regular aerobic exercise.

Drug intervention: If blood lipids still significantly increase after lifestyle adjustments, doctors can add statins or beta lipid-lowering drugs, and pay attention to drug interactions.

2. Monitoring and treatment of proteinuria

Regular screening: It is recommended to test urine routine or urine microalbumin/creatinine ratio every 3-6 months.

Graded management: Mild proteinuria (<1g/24h) can continue to be treated with medication and strengthened monitoring; Moderate to severe proteinuria (≥ 1g/24h) requires evaluation of renal function, consideration of dose reduction or dressing change, and combination of ACEI/ARB drugs to reduce urinary protein.

3. Identification and response to interstitial pneumonia

High vigilance: When dry cough, progressive dyspnea, low-grade fever or fatigue occur, drug-induced interstitial pneumonia should be suspected.

Emergency treatment: Once diagnosed or highly suspected, sirolimus should be immediately discontinued and glucocorticoid therapy should be given, along with high-resolution chest CT and pulmonary function examination.

4. Prevention of delayed wound healing

Preoperative preparation: It is recommended to stop using sirolimus for at least one week before elective surgery (subject to evaluation by a doctor).

Postoperative care: closely observe the healing of the incision and keep the wound clean and dry; If the healing is poor, timely medical attention should be sought. After ruling out infection, the doctor can evaluate whether to reduce or suspend medication.
