Core precautions for taking Seratinib: monitoring of the four major toxicity risks

During the use of Seratinib, active monitoring should be maintained for the following four types of serious adverse reactions. All warning signals must be responded to immediately and symptoms should not be waited for to resolve on their own.

1、 Gastrointestinal toxicity (diarrhea, nausea, vomiting)

Monitoring indicators: Record the frequency and characteristics of bowel movements daily, and observe whether there is bloody stool or dehydration symptoms (dry mouth, low urine).

Intervention timing: If diarrhea occurs at least 6 times within 24 hours, or if vomiting causes inability to eat or drink water, stop taking medication and contact a doctor. Do not use antidiarrheal drugs to cover up the condition on your own.

Dietary coordination: Adhere to a low-fat and low residue diet, avoid raw, cold, and gas producing foods, and reduce mechanical stimulation to the intestines.

2、 Hepatotoxicity (elevation of transaminase and bilirubin)

Mandatory testing: Liver function tests, including ALT, AST, and total bilirubin, are conducted before treatment, every 2 weeks (lasting for 2 months), and monthly thereafter.

Discontinuation threshold: If ALT/AST is greater than 3 times the normal upper limit and accompanied by bilirubin greater than 2 times the upper limit, or ALT/AST is greater than 5 times the upper limit, immediate discontinuation and hospitalization are required. After recovery, the dose should be reduced.

Self observation: Pay attention to yellowing of the skin or sclera, dark urine, and pain in the upper right abdomen. If any of these symptoms occur, draw blood for a follow-up examination.

3、 Interstitial lung disease (ILD)/pneumonia

Warning symptoms: sudden or aggravated dry cough, chest tightness, shortness of breath, fever (no evidence of infection).

Urgent action: If any of the above new respiratory symptoms occur, stop medication immediately and seek emergency medical attention for high-resolution CT examination.

Diagnosis and treatment: Once diagnosed with ILD, it is necessary to permanently discontinue Seratinib and switch to hormone therapy, which cannot be challenged again.

4、 High blood sugar (elevated blood sugar)

Baseline assessment: fasting blood glucose and glycosylated hemoglobin were detected before treatment, and diabetes patients need to optimize the hypoglycemic program in advance.

Regular follow-up: Fasting blood glucose should be measured once a week in the first month of treatment, and once a month thereafter.

Treatment standard: If fasting blood glucose continues to be>13.9mmol/L (250mg/dL), Seratinib should be discontinued and insulin therapy should be initiated. After the control is stable, the dosage should be reduced and restored.

5、 Mandatory medication regulations

During the medication period, it is absolutely forbidden to consume grapefruit or drink grapefruit juice. If any adverse reactions>grade 2 occur, the medication should be suspended until it drops to ≤ grade 1 before restarting at a low dose.
