Gilteritinib contraindications and core medication risks
Gilteritinib poses significant risks in specific patient populations and combination therapy scenarios, and should be strictly avoided or particularly cautious. Clarifying the following contraindications and medication restrictions is the fundamental prerequisite for ensuring treatment safety.
1、 Absolute taboo group
1.1 Patients with severe liver dysfunction: It is contraindicated for patients with severely insufficient liver function reserves, as drug metabolism mainly relies on the liver, which may lead to toxic accumulation.
1.2 Patients with severe renal insufficiency: Patients with severe renal dysfunction (such as those requiring dialysis) are prohibited from using it, as there is a lack of sufficient safety data to support its use in this population.
2、 The risk of combined use of potent CYP3A4 inducers
2.1 Risk mechanism: Strong CYP3A4 inducers (such as rifampicin and phenytoin sodium) can significantly reduce the blood concentration of Gilteritinib, which may lead to a significant decrease in efficacy or even failure.
2.2 Treatment principle: Co use of such drugs is prohibited during the treatment period. If the patient is currently using Gilteritinib, it should be discontinued before starting treatment (with sufficient washout period) and replaced with non interacting alternative drugs.
3、 Special caution is required when using medications in combination
3.1 Strong CYP3A4 inhibitors: such as ketoconazole and clarithromycin, may increase exposure to Gilteritinib and increase toxicity risk. If combined, adverse reactions should be closely monitored.
3.2 P-gp (P-glycoprotein) substrate drugs: Co administration may alter their distribution, caution should be exercised and medication should be adjusted according to medical advice.
