Dotinurad contraindication population: Notice on history of severe renal insufficiency and urinary tract stones
Some individuals have a significantly increased risk of using Dotinurad due to their own conditions, with severe renal insufficiency being a clear contraindication and a history of urinary tract stones requiring special caution. The following are the core precautions.
1、 Severe renal insufficiency (eGFR<30mL/min/1.73m ²)
Absolute prohibition: This product lacks safety data in such patients and may increase the risk of kidney injury, therefore its use is prohibited.
Assessment must be conducted before medication: eGFR should be tested before initiating treatment to confirm ≥ 30mL/min/1.73m ² before use.
Monitoring during treatment: If eGFR continues to decrease to<30mL/min/1.73m ² during treatment, the medication should be stopped immediately and a nephrologist should be consulted.
2、 Notice for patients with a history of urinary tract stones
Not an absolute contraindication, but caution should be exercised: those with a history of urinary tract stones are not completely prohibited from using it, but medication should be taken under close monitoring by a doctor.
Pre medication evaluation: It is recommended to undergo urinary ultrasound or CT examination to determine whether there is a current risk of stones or significant obstruction.
Adequate hydration: During treatment, daily water intake should be at least 2 liters, evenly distributed to avoid excessive concentration of urine.
Avoid combination therapy with stone causing drugs: If unnecessary, avoid using high-dose vitamin C, calcium supplements, or thiazide diuretics simultaneously to reduce the risk of stone recurrence.
Seek medical attention immediately if symptoms appear: If you experience lower back pain, hematuria, or painful urination during medication, you should suspend medication and seek medical attention as soon as possible.
3、 Other groups to be aware of
Severe liver dysfunction: Lack of research data, not recommended for use.
Pregnant and lactating women: Safety unknown, should avoid use.
Acute gout attack period: Although not contraindicated, it is recommended to control the attack before starting uric acid lowering treatment to reduce confounding factors.
