Sorafenib: Usage, Dosage, Efficacy, and Precautions
Sorafenib is an oral multi-target kinase inhibitor, and standardized medication, clear efficacy evaluation indicators, and implementation of safety monitoring are the three pillars of treatment management. The following are the core points.
1、 Usage and dosage
The recommended dosage is 400mg per dose (two 200mg tablets), taken twice a day on an empty stomach or with a low-fat meal.
Should be swallowed whole, not crushed or chewed; If missed, take the next dose at the regular time and avoid taking double the dose.
When toxic reactions occur, gradually reduce the dosage according to the instructions (such as reducing to 400mg per day or 400mg every other day) or suspend treatment.
2、 Efficacy evaluation
Mainly based on imaging examinations (such as CT or MRI), evaluate tumor size and new lesions every 6-8 weeks.
Simultaneously monitor the trend of changes in tumor markers (such as AFP) as auxiliary reference indicators.
Clinical benefit assessment also includes symptom improvement (such as pain relief) and maintenance of physical condition.
3、 Key precautions
Before and during treatment, liver function (ALT, AST, bilirubin) should be tested monthly. If the transaminase level exceeds three times the normal upper limit, it should be paused.
Monitor blood pressure weekly, and if it remains above 140/90mmHg, initiate antihypertensive treatment.
Observe the skin daily, strengthen moisturizing when there is redness or peeling on the hands and feet, and avoid friction; Timely fluid replacement and use antidiarrheal medication when experiencing diarrhea.
