Ribociclib: Usage, Dosage, Efficacy, and Precautions

Ribosilide is a CDK4/6 inhibitor for advanced or metastatic breast cancer with HR positive and HER2 negative. Standardized medication is the foundation for ensuring efficacy and safety.

1、 Standard usage and dosage

1. Recommended solution

Used in combination with letrozole or fluvoxetine. Oral administration, once a day, 600mg each time (i.e. three 200mg film coated tablets), continuously taken for 21 days, stopped for 7 days, for a total of 28 days as a complete cycle.

2. Medication method

It is recommended to take it with or after meals to alleviate nausea. Take the medication at a fixed time every day, and swallow the tablet as a whole. Do not chew or crush it. If missed, vomited, or experienced diarrhea, no further supplementation will be given on the same day, and the next day will continue according to the plan.

Significantly prolong the progression free survival (PFS) of patients and reduce the risk of disease progression.

3、 Medication precautions

1. Prohibited population

Individuals with an absolute neutrophil count below 1.0 × 10 ⁹/L are prohibited from using it. Child Pugh C grade (severe liver damage) patients are contraindicated. Individuals with a history of severe arrhythmia or QTc interval>480 milliseconds are prohibited from using it.

2. Situations requiring monitoring and adjustment of dosage

Mild to moderate liver damage or elderly patients do not require adjustment, but require close monitoring. When used in combination with strong CYP3A4 inhibitors such as clarithromycin and itraconazole, the dose should be reduced to 400mg/day. Avoid consuming grapefruit during treatment. If there are ≥ grade 3 adverse reactions, the treatment should be suspended or reduced to 400mg/day.

3. Important risk monitoring

Blood routine, liver and kidney function, and electrocardiogram will be rechecked every 2 weeks for the first 2 cycles, and monthly rechecks will be conducted for subsequent cycles. If QTc interval is prolonged or liver enzymes are significantly elevated, medication should be suspended and evaluated.
