Venetoclax administration guide: with meals? Dose escalation and missed dose management
Venetoclax must be taken with a high-fat and high calorie meal, with doses increasing from 20 → 50 → 100 → 200 → 400mg per week. If missed for more than 8 hours, it should be skipped and resumed the next day. All adjustments must be made under the guidance of a doctor and cannot be changed without authorization.
1、 Taken with meals (requires high-fat and high calorie foods)
Must be taken with meals, and should choose meals containing about 30-50g fat and 500-600 calories (such as fried eggs, whole milk, butter bread), which can significantly improve drug absorption.
Avoid taking medication on an empty stomach or only with light meals, as it may affect blood drug concentration.
Take medication at a fixed time every day (such as breakfast) to maintain consistency.
Swallow the whole piece, do not crush, chew or pry open.
2、 Weekly dose escalation plan (5-week plan)
Week 1: 20mg/day
Week 2: 50mg/day
Week 3: 100mg/day
Week 4: 200mg/day
Week 5 and beyond: 400mg/day (target maintenance dose)
Each dose adjustment is made on the first day of the new week.
If serious adverse reactions occur during the escalation period, the previous tolerated dose should be paused or recalled, and the decision to continue escalation should be evaluated after recovery.
3、 Handling of missed services
3.1 Supplementary Service Rules
If the missed time is less than 8 hours, take the medication immediately and continue taking it at the original time the next day.
Missed dose time ≥ 8 hours: skip this dose and take according to the original plan the next day. Do not double the dose.
3.2 Vomiting Management
If vomiting occurs within 30 minutes after taking the medication, it can be supplemented once; If it exceeds 30 minutes, there is no need to retake it, and it will continue according to the next day's plan.
