Precautions for taking Sildenafil: monitoring of liver toxicity, changes in blood lipids, and drug interactions
Regular monitoring of liver function and blood lipids, as well as attention to potential drug interactions, is necessary during the treatment period with Sladapper. Once there is a significant increase in liver enzymes, the medication should be stopped promptly and evaluated; When using other medications in combination, one should proactively inform the doctor to ensure medication safety.
1、 Hepatotoxicity monitoring
1.1 Monitoring frequency
Before treatment: Check baseline liver function (ALT, AST, total bilirubin, alkaline phosphatase).
During the treatment period: once a month for the first 3 months, and once every 3 months thereafter.
1.2 Exception Handling
ALT/AST>3 × ULN: Suspend medication and have a follow-up within one week.
ALT/AST>5 × ULN: Stop medication immediately and investigate other causes of liver injury.
If transaminase elevation is accompanied by total bilirubin>2 × ULN, permanent discontinuation of medication is required.
2、 Monitoring of blood lipid changes
2.1 Monitoring indicators
Total cholesterol (TC), low-density lipoprotein (LDL-C), high-density lipoprotein (HDL-C), and triglycerides (TG) were measured every 3 months before and after treatment.
2.2 Management Suggestions
If LDL-C increases by ≥ 30% from baseline, statin or ezetimibe therapy should be initiated or adjusted.
Suggest strengthening dietary control (reducing saturated fat intake) and regular exercise simultaneously.
3、 Drug interactions
3.1 Medications to be used in combination with caution
OATP1B1/1B3 inhibitors (such as cyclosporine and gefitolozil): can increase the blood concentration of sorafenib, and adverse reactions should be monitored when used in combination.
Strong CYP3A4 inducers (such as rifampicin and phenytoin sodium): may reduce the efficacy of Sildenafil and need to be evaluated for dose adjustment.
3.2 Medications that should be avoided in combination
Bile acid binding resin (such as colexamide): It should be taken at least 4 hours apart from Sildenafil to avoid absorption effects.
Warfarin: Co administration may affect INR, and monitoring frequency needs to be increased.
3.3 Traditional Chinese Medicine and Health Products
Avoid using St. John's wort to avoid inducing metabolism and reducing blood drug concentration.
