Finarenone is a new type of non steroidal mineralocorticoid receptor antagonist, which is mainly used to treat type 2 diabetes related chronic kidney disease. The following provides a detailed explanation from three aspects: usage, dosage, renal function monitoring, and precautions. The content is strictly organized according to the drug specifications.

1. Usage and dosage

(1) Standard dose: The initial dose is 10mg or 20mg orally once daily, adjusted according to eGFR and blood potassium levels.

(2) Dose adjustment: 20mg can be used when eGFR ≥ 60mL/min/1.73m ²; 10mg is recommended when eGFR 25-60mL/min/1.73m ²; eGFR<25mL/min/1.73m ² is prohibited.

2. Renal function monitoring

(1) Baseline assessment: eGFR and blood potassium levels must be tested before medication.

(2) Regular monitoring: Renal function and blood potassium should be rechecked after the 4th and 12th week of treatment and dose adjustment, and monitored every 3-6 months thereafter.

3 Precautions

(1) Contraindications: Not recommended for patients with severe liver dysfunction (Child Pugh C grade), adrenal insufficiency, and hyperkalemia (>5.0 mmol/L).

(2) Drug interactions: Avoid using potent CYP3A4 inhibitors or inducers in combination, and strengthen blood potassium monitoring when used in combination with ACEI/ARB.

(3) Special populations: contraindicated during pregnancy, weighing the pros and cons during lactation, elderly people do not need to adjust dosage but need to strengthen monitoring.
