Futibatinib: Guidelines for irreversible FGFR inhibitors in the treatment of cholangiocarcinoma
Futibatinib is an irreversible FGFR inhibitor primarily used to treat patients with unresectable biliary tract cancer carrying FGFR2 gene fusion or rearrangement. The following provides a detailed explanation from three aspects: indications, usage, dosage, and precautions.
1. Indications
Succinic acid tasugtinib (Fubatinib) is suitable for the treatment of unresectable biliary tract cancer patients who have progressed after chemotherapy and carry FGFR2 gene fusion or rearrangement.
2. Usage and dosage
(1) Standard dose
Adult patients take 160 milligrams orally daily, divided into two doses (80 milligrams each time), and continue treatment until disease progression or unacceptable toxic reactions occur.
(2) Dose adjustment
Dose adjustment based on the severity of adverse reactions: first dose reduction to 120mg/day, second dose reduction to 80mg/day. If further dose reduction is required, the medication will be permanently discontinued.
3. Precautions
(1) Hepatotoxicity monitoring
Regular monitoring of liver function (ALT/AST/bilirubin) is required, and dosage adjustment or discontinuation of medication is necessary when hepatotoxicity occurs.
(2) Risk of bleeding
Serious bleeding events may occur, and signs of bleeding need to be monitored and the risk of using antithrombotic drugs evaluated.
(3) Infection prevention
Patients with increased risk of infection should consider preventive measures, including vaccination and antibiotic prophylaxis.
(4) Fertility related risks
Women of childbearing age should take effective contraceptive measures and avoid pregnancy during treatment and within one week after discontinuing medication.
