The management of side effects of Pralsetinib should focus on rash, pancreatitis, and hypertension. The following are specific measures and precautions to help patients take medication safely.

1. Skin rash management

Rash is a common side effect of Punitinib, typically manifested as redness, itching, or dryness of the skin. Mild rash can be relieved with moisturizer and avoid using irritating skincare products. If the rash worsens or blisters appear, immediate medical attention should be sought, and medication dosage may need to be adjusted or antihistamines used.

2. Response to pancreatitis

Pancreatitis may manifest as severe abdominal pain, nausea, or vomiting. Patients need to regularly monitor their blood lipids and pancreatic enzyme levels. When symptoms appear, medication should be stopped immediately and imaging examinations should be performed. The recovery medication needs to be evaluated by the doctor, and the dosage may need to be reduced.

3. Hypertension control

Baseline blood pressure should be assessed before treatment and monitored weekly during the treatment period. When hypertension (≥ 140/90mmHg) occurs, ACEI or ARB antihypertensive drugs are preferred. Grade 3 hypertension (≥ 180/110mmHg) requires medication to be temporarily suspended and reduced for recovery after control.

(1) Drug intervention

Select intervention measures based on blood pressure grading: Grade 1 (140-159/90-99mmHg) can continue medication; Grade 2 (160-179/100-109mmHg) requires the addition of antihypertensive medication; Level 3 requires immediate discontinuation of medication and intravenous hypotension.

(2) Lifestyle adjustment

Limit sodium intake (<6g per day), maintain BMI<25, and engage in moderate intensity exercise for ≥ 150 minutes per week. Avoid NSAIDs and cold medicines containing ephedrine.

4. Comprehensive monitoring requirements

Before treatment, liver function, blood routine, and electrocardiogram should be checked. Recheck every 2 weeks for the first 3 months, and once a month after stabilization. Symptoms such as visual impairment, persistent headache, or chest tightness require immediate medical attention.

5. Precautions for special populations

Pregnant women are prohibited from using dual contraception during their reproductive age. Breastfeeding should be suspended during lactation. Elderly patients do not need to adjust the dosage, but need to strengthen blood pressure monitoring. Patients with liver dysfunction need to assess the benefit risk ratio.
