Tarlige, as a neuropathic pain treatment drug, is mainly contraindicated for patients with special physiological conditions, specific diseases, and high-risk drug interactions. The following provides a detailed explanation from three aspects: absolute contraindications, relative contraindications, and situations requiring cautious use, to help patients avoid medication risks.

1. Absolute taboo group

(1) Patients with severe renal insufficiency: creatinine clearance rate<15mL/min or dialysis patients should not use it, as drugs are mainly excreted through the kidneys, which may lead to accumulation poisoning.

(2) Pregnant women: Animal experiments have shown a risk of abnormal embryo development, and although human data is limited, strict contraception is recommended.

2. Relatively taboo groups

(1) Moderate renal impairment: The dosage needs to be adjusted to 50% of the regular dosage, and creatinine levels should be monitored weekly.

(2) Liver dysfunction: Children Pugh B/C grade patients should reduce their dosage by 25% -50%, as the CYP2D6 metabolic pathway may be affected.

3. Use with caution

(1) Elderly patients: The starting dose for those over 65 years old should be 50mg/day, slowly titrated to the effective dose.

(2) Depressed/suicidal individuals: It is necessary to strengthen emotional monitoring as this medication may exacerbate mental symptoms.

(3) Combination therapy: When used in combination with CNS inhibitors (such as opioids), the dosage should be reduced by at least 30% to prevent synergistic inhibitory effects.

4. Special precautions

(1) Allergic reactions: People who are allergic to gabapentin drugs may experience cross allergy.
