Salofalk, as a 5-aminosalicylic acid preparation, has significant anti-inflammatory effects on mild to moderate ulcerative colitis and Crohn's disease. Discontinuation should be gradually reduced to avoid recurrence

1. Evaluation of treatment efficacy

(1) Ulcerative colitis

The effective rate of induction remission period is about 60-80%, and maintenance treatment can reduce the recurrence rate by 50-70%. Symptom improvement usually occurs within 2-4 weeks of medication and requires continuous medication to maintain efficacy.

(2) Crohn's disease

The effect on colonic Crohn's disease is significant, with a lower response rate to ileal lesions. Maintenance therapy can prolong the remission period by more than 1 year and requires regular endoscopic evaluation.

2. Medication during pregnancy

(1) Risk level

Pregnancy medication belongs to Class B, and animal experiments have not shown teratogenicity, but human data is limited. Patients with moderate to severe activity need to weigh the impact of disease activity on pregnancy.

(2) Principles of medication use

Try to avoid using it during the first 3 months of pregnancy, and when it is necessary to use it in the middle and late stages, it is recommended to use the lowest effective dose and strengthen fetal monitoring.

3. Medication during lactation

(1) Drug secretion

Approximately 0.1% of the administered dose is secreted into breast milk. It is recommended to suspend breastfeeding during lactation, especially for premature infants or infants with hyperbilirubinemia.

(2) Resume feeding

After stopping the medication, breastfeeding can be resumed after a half-life of 3 hours (approximately 36 hours), during which the baby's gastrointestinal symptoms need to be monitored regularly.

4. Medication for children

(1) Applicable age

Children over 5 years old can use it, but the dosage needs to be adjusted according to their weight. The safety and efficacy of children under 5 years old have not yet been established.

(2) Dose calculation

Administer in batches of 20-30mg/kg/day, with a maximum dose not exceeding the adult dose. Special oral suspension is required to ensure accurate dosage.

5. Medication for the elderly

(1) Dose adjustment

Patients over 65 years old may experience a 25% reduction in initial dosage due to natural decline in kidney function, which may increase the risk of drug accumulation.

(2) Monitoring focus

It is necessary to strengthen renal function monitoring, and it is recommended to check serum creatinine every month. If dehydration symptoms occur, seek medical attention immediately.

6. Liver dysfunction

(1) Mild to moderate damage

Child Pugh A/B grade patients do not require dose adjustment, but transaminase levels need to be monitored every 2 weeks to stabilize.

(2) Severe damage

Child Pugh Class C patients are contraindicated as it may increase the risk of hepatic encephalopathy.

7. Renal insufficiency

(1) Mild to moderate damage

The patient dose of eGFR30-89mL/min was halved, and the dosing interval was extended to 12 hours.

(2) Severe damage

EGFR<30mL/min or dialysis patients should not use it, as it may cause crystalline kidney disease.
